For most patients, the recommended dose is 25 mg every two weeks intramuscularly. The following conversion schedule should be considered for patients already receiving a fixed dose of oral risperidone for two or more weeks. Patients treated with doses of 4 mg or less of oral risperidone should receive 25 mg of RISPERDAL CONSTA, whereas, for patients treated with higher oral doses, the higher dose of RISPERDAL CONSTA 37.5 mg should be considered.

For patients not currently taking oral risperidone, oral pre-treatment should be considered when choosing the i.m. starting dose. The recommended starting dose is RISPERDAL CONSTA 25 mg every two weeks. In patients treated with higher oral doses, the higher dose of RISPERDAL CONSTA 37.5 mg should be considered.

After the first injection of RISPERDAL CONSTA, sufficient antipsychotic coverage with oral risperidone or the previously used antipsychotic should be ensured for the first three weeks (see section 5.2).
